falsified medical records?

My husbabd and I took our 7 weeks old 6 weeks premature baby to the emergency room here in Arizona. Physiacians assistant who attended my daughter came in its fourth review once for 2 min then some people came and told the AP that the mental health clinic was there to see it came out saying that he back were never high on my daughter without testing it. She died 2 days later of pneumonia. When asked our daughters medical records are sometimes in reviews and revisions of the system that was never held my daughters stateing variety of symptoms did not exist when you can clearly see the triage report did. . Can anyone give me some advice about it. We are consulting a lawyer, but I'm angry and confused about what to do. Any advice is welcome constuctive

I'm sorry about your loss. We currently do not do anything without talking to your lawyer. If you accidentally say something that can be used against them. Hopefully your lawyer will talk everyone who was on the floor at that time. If there is a thin can remember the names, times, things happen that you can write to raise them. You can also request an investigation be held. There must be at least a decent person on the ground that will tell the truth. My prayers are with you and your husband and good luck.
